Talk to Urologist on Varicocele, Spermatorrhea, Low Sperm Count

Hi,
I have done a semen analysis and found that sperm count is extremely low less than 5 million/ml and motility of sperm is just 5% so after a scrotal ultrasound it relieved me that i have varicocele on both sides. I had a constipation from last 2 years and it cause sperm leakage whenever i have done a poo so i guess it is a sign of spermatorrhea. Now i am very confused which thing is the cause of other disease and what should i treat on priority base. Please Help!
