Talk to Urologist on Spermatorhea+Gynecomastia

I am unmarried. I have issue of Gynecomastia and also when I urinate, semen (White Fluid) comes out with urine. Sometimes it comes out at the start of urine but usually, it comes at the end of the urine with irritation. Sometimes Transparent fluid also comes out when thinking sexually. I used to masturbate often at college and school level but I do it now after months. But when a month or two passed by without masturbation, I feel little pain and itching in my testicles(also itching in urethra) and then after some time (next day usually) Semen (White) comes out and pain and itching goes away. I think about sex a lot also. My Penis has become very weak and thin. I got morning erections but erection goes away quickly.During High School,I had some severe headaches,diagnosed shortsightedness,wear glasses now. Recently I have done some tests as follows: (Weight: 60 Kg)(Testosterone: 334.0 ng/dL)(Prolactin: 5.23 ng/ml)(TSH:2.61 mlU/L)
