If test is fresh it's ok otherwise get fresh semen Analysis done

Your report is not good, morphology is too much disturbing. You have long abstinence period which should not be more than five days. Repeat analysis, if report is so, have a ultrasound and color doppler of scrotum for testicular size , flow and varicocele
