if he is coughing sputum then get sputum for AFB smear, AFB CULTURES, and genexpert. more over did he experience t.b in past?? get the above test done.
if not coughing up sputum get pleural effusion aspiration and get pl. effusion DR ,pl.effusion cytology ,pl.effusion AFB geneexpert ,Cultures.
see an expert advise both pulmonologist and cardiologist. as the age is such where all the factors are important to consider. may Allah relief his condition .

as there is bilateral effusion
first get an echo of heart and pro bnp levels to rule out cardiac failure
meanwhile you can go for sputum gene xpert and bacterial culture

you need to have sputum genexpart plus echocardiography to rule out cardiac issue ...
