aoa dear.i can understand your concern regarding your brother mental state. In some psychiatric disorders patients looses touch with reality and is not well aware of his deteriorating mental health due to his distorted thinking process. In such situation responsibility of treatment is shifted to care takers of the patient. So you are advised that you should consult psychiatrist and discuss treatment plan. There are many ways to manage the situation even if patient refuse treatment

Wsalam, the patient actually does not realise that he has any issue , taking overdoses of medicine is a common thing for him, the problem is that even parents dont realize the severity and they think ignoring the issues is the solution which is actually not, and the psychiatrist said nothing much can be done without the patient himself realizing that he needs to come out of it or take any help

What ever the issue (suffering) from any illness, he needs treatment to lesser his and his family pains . So medicine May start to give him insight and willingness to visit therapist (psychiatrist) for assessment of treatment too.

He does not take medicine without his own will , and he is an adult medicine cant be given forcefully either, we are in a very painful situation because parents dont realize the seriousness of his situation so we have no support from the adults of family either...
