I am noticing orange/red stains in my 6 months old baby boy's diapers since yesterday. Stains are at urine side of diaper. I am very worried about it outer part of penis is looking fine. What could be the reason? He is on breastfeeding and had mango puree yesterday afternoon.

Assalam o alaikum.. You have noticed this for the first time. This needs to be observed., if child has no other symptoms. You may get urine complete examination to rule out any underlying condition and may contact again.

Should i go for urine D/R or urine R/E test? Are both different?

Both are same. You can wait and if observed for second time, go for it.
