How extensive is the napkin area rash? You may use petroleum jelly liberally in the area so that urine or perhaps acid stools do not come in contact with skin to cause local irritation and rash. Keep the baby exclusively on breastfeeding. Spotting of diaper with passing the winds is not anything serious.

So called breast milk jaundice, if that is what your son has, may last up to 8-12 weeks of life. However, a jaundice persisting beyond two weeks in a full term infant may need some tests to rule out any cause of concern. Placing a baby in the sun does not do any good.

This gestational age makes your baby technically preterm. For a preterm baby jaundice is termed as persistent or prolonged after 3 instead of two weeks. I would rather get a baseline test done to be sure we are not missing anything. Serum Bilirubin Levels, Total and Direct will guide us how to proceed further.
