Talk to Gynecologist on Menstrual Cycle Issue/pregnancy

I am 6month married diagnosed with pcos in june,taken primlout N for missed periods after doctors recomendation,get my periods on 24 july. After than i get spots on 19aug on and off it continues till 21 with alot menstrual cramp,fever,lose motion,back pain.but no proper period or blood flow only spots. Now m feeling nusea, abdominal pain,back pain. Should i go for physical examination or pregnancy test? Or its normal harmonal change due to primlout n.?
