Talk to General Practitioner on Perianal Hematoma

I had pain and fever for the last 7 days. Went to the doctor on the third when fever didn't go down. During physical exam I was diagnosed with perianal hematoma (healing stage) and enteric fever.

Went back and kept taking Panadol and antibiotics and started using polyfax plus.

A couple days later the pain worsened. And a day later another painful lump formed as shown in the image (at the very end). This is acutely painful and I can't move at all.

As you can see (if you zoom image) there are some white pimple like thing in the red area above the lump. And then there are white lines at the top. The tests for off/igm returned negative.

Now I am in acute pain and fever hasn't come down even now.

Can someone please help me understand what this is? And how to go about fixing it?
