Talk to General Physician on Headache( Dizziness)

My wife has had a headache for four or five years, in the beginning it was occasional But over the last two or three months, it has intensified In the first phase, severe dizziness occurs, followed by continuous pain for several hours This pain spreads to the temple & lower back. Both ears would be closed . The brain becomes very sensitive. Even the slightest sound of something like tearing the brain. Doctors at the local hospital only give a panadol or a painkiller but the matter seems very complicated. Please guide us. We are very worried about which doctors should we consult Thanks .
