Talk to Ent Specialist on Itching In Ear

Dear Doctor, I have itching and dryness in ear for long time but these days it has increased and I have noticed blackish stuff on earbuds (cotton tips) when cleaning my ear like fungus (mold on bread). Beside this I have issue in toenail and itching on left foot ankle (pictures attached). Additionally, I have bump on tip of tongue which recurs after week or so on same place (internet picture is attached). Itching on foot ankle is also recurring after few months or so on same place of ankle. Toenail issue is also recurring in same toenail. First of all, are these all problems connected with each other ? Secondly, suggest any prescription for Ear so that I may buy from any Farmacy because I am currently in Barcelona, Spain and don't want to visit hospital in this situation of COVID-19.

Note: I use Terbiderm anti-fungal cream on foot itching and Dektarin Oral Gel for tongue issue.
