If it's very old scar like more than 1 or 2 mo then consult plastic surgeon.. Any cream application won't affect much once scar becomes older..
If it's fresh scar, u can apply contactubex gel twice a day

I Don't see a need of plastic surgery plus pic isn't of IDEAL quality... as suggested book appt via Marham so can discuss properly n guide abt options...options % improvement n time frame & wht creams that can blend the scar/ and PIH that has started from edges, Regards
