Red Patch On Scalp (itches And Aches)

Dear docs, recently i notices i m losing lot of hairs from the center of my scalp. and there i see a large red colored patch which causes too much irritation and even it aches. kindly suggest me some treatment. i even get my scalp bald and now i can see its getting really worse.
