This condition is called Pericoronitis.
Aqal darh ke upar se soft tissue ko nahee cut karna hota, wahan uss jaga par irrigation with normal saline hotee hai. Irrigation simple kaam hai. Aap ko pain nahee ho gaa. Hum anesthesia laga kar syringe use kar ke pressure se wahan normal saline flush karein ge. Issue foran resolve ho jaye ga.
Reh gaya aap ka sawal tu agar koi aap ke baat maan kar upar soft tissue (gosht) ko kaat deta hai yanee Operculectomy tau yeh aik waqtee hal hai.
Aap irrigate karwain kionkay wahan bacteria aur pus jamaa ho chukee hai aur saath 5 din ya 3 din antibiotic ka course ho gaa.

In chronic pericoronitis cases best treatment options are operculactomy along with medications.Moreover clinic sitting will decide the best treatment plan along with severity of disease and early possible outcomes to resolve patient complain with help of radiographs .With proper diagnosis treatment will proceed further.

Well, coming to the fact that I mentioned the name of book was to emphasize the temporary and transient relief provided by operculectomy. Again I am talking with a reference, there are 3 treatment options for pericoronitis in general:
1. Extract the involved tooth
2. Operculectomy
3. Extract the opposing maxillary tooth to relieve pain during occlusion.
The first option, is the final treatment but CANNOT be done in the presence of active pericoronitis.
Second option is a temporary relief and the operculum will always tend to grow back.
Third option may be too invasive, suppose the patient wants a maxillary prothesis and you need to use 18 or 28 as abutments.
So we have to resolve pericoronitis first then go for option 1 (extraction). That said, irrigation and antimicrobial prophylaxis are the only two non-invasive techniques. Also, a history of psychological stress must be taken and referral to a psyhiatrist if psychotic symptoms or depression is seen or patient complains of it.
Lastly, I can define practice but cannot define practice without a sound knowledge base (EBD).

To know the reason that why the third molar is not erupting ,a periapical X-ray needs to be taken.Not the overhanging gum but the position of the tooth in the bone is important.After evaluating X-ray ,and examing clinically it would be decided to extract the tooth.

main reason is food impaction between gum and partialy eruptive tooth. try to keep it clean with brushing. warm saline gargles and occasional use of enziclor Mouthwash.

WS.. you are suffering form pericoronitis. Take Tab Denzn DS twice a day for 3 days. Apply enziclore gel twice a day for 2 weeks. And use use Niflam mouthwash for 7 days. InShaAllah it will get better.

You are right and extraction is the main solution but we cannot do extraction on the face of pericoronitis. Let the inflammation resolve first thats why I suggested irrigation+antibiotics.
